To help you complete the Kentucky Tax Registration Application, we suggest you have the following information ready and available:

• The legal name of the business and, if applicable, its DBA (doing business as) name.
• The Federal Employer Identification Number (FEIN) of the business. If you do not have a FEIN, apply for one online with the Internal Revenue Service at irs.gov.
• The legal structure of the business. For example, a corporation or limited liability company (LLC).
• The federal tax election of the business. For example, taxed as a partnership or corporation at the federal level.
• The location of the business and the mailing address for each tax account.
• All appropriate responsible parties of the business, including each person's legal name, residence address, phone number, and Social Security number.
