To help you complete the Kentucky Tax Registration Application, we suggest you have the following information ready and available:
The legal name of the business and, if applicable, its DBA (doing business as) name.
The Federal Employer Identification Number (FEIN) of the business. If you do not have a FEIN, apply for one online with the Internal Revenue Service at irs.gov.
The legal structure of the business. For example, a corporation or limited liability company (LLC).
The federal tax election of the business. For example, taxed as a partnership or corporation at the federal level.
The location of the business and the mailing address for each tax account.
All appropriate responsible parties of the business, including each person's legal name, residence address, phone number, and Social Security number.
